Atypical Femur Fracture after Spine Surgery in Elderly Patients with Long-term Bisphosphonate Treatment: Two Case Reports and Review of Literature

Abstract

|
A 74-year-old female patient with a history of prolonged alendronate use presented with lower back pain and right lower extremity radiating pain. After surgery for a ruptured disc, the patient complained of bilateral anterior thigh pain, which eventually prevented her from walking. Radiographs revealed atypical femur fractures (AFFs) in both femurs. A 79-year-old female patient presented with severe lower back pain and both lower extremity radiating pain. The patient received lumbar epidural neuroplasty for relief of symptoms, but left thigh pain persisted. Symptoms worsened after five days and plain radiograph of the left femur revealed a diaphyseal femur fracture. We present two cases of patients presenting with atypical femur fracture after spinal surgery and review literature.
